Chronic Disease Management Plans

Chronic Disease Management (CDM) items allow eligible patients to have longer, structured appointments fully bulk-billed. These plans provide a framework for ongoing care and, when needed, referrals to allied health services (e.g. physiotherapy, podiatry, dietetics).

Chronic Disease Management Plans

Eligibility

You must have a chronic or terminal medical condition that has been (or is likely to be) present for 6 months or longer.

Examples include: diabetes, heart disease, asthma, arthritis, cancer, chronic kidney disease, or musculoskeletal disorders.

Patients with complex care needs requiring input from multiple providers may also be eligible for Team Care Arrangements (TCA).

Benefits

A written care plan prepared with your GP.

Up to 5 Medicare-subsidised allied health visits per year.

Regular reviews (at least every 3–6 months).
